Reablement Specialists
Health Vision is an experienced re-ablement provider for
Westminster Social Services and has successfully reduced the
support requirements for a number of service users. Our Care
Supervisors have the experience and the skills to monitor progress
and manage client confidence, physical limitations and motivation
so that achievable gains in independence are sustainable.
Health Vision believes the key to successful re-ablement is to
develop an effective and realistic outcomes focussed care plan with
the full participation of the service user. The service user should
be encouraged to set objectives for themselves that include the
gaining of new skills, or regaining skills that may have been
lost.
Health Vision's Care Supervisors are trained in the facilitation
of, development and setting of goals which are Specific,
Measurable, Achievable, Realistic and Time bound (SMART), and
currently use this methodology, with the Service User's agreement,
in existing services.
As our carers are a key resource for implementing re-ablement
strategies, their experience, education and understanding is
critical for successful outcomes. In addition to a robust care
plan, the Health Vision Care Supervisors provide ongoing support
and guidance so that timely changes to routines and exercises can
be introduced when required.
Key aspects of re-ablement are:
- Encouraging service users to look after their own health and
well being as much as possible
- Training of Health Vision staff and regular communication of
the principles of re-ablement. This means a concentration on
achieving particular outcomes for people, rather than simply
completing tasks.
- Assisting service users to lead an independent life, with
confidence in their own abilities, thus reducing reliance on
others.
In my experience Health Vision provide a level of
service which exceeds that which other care agencies provide. They
actively help people to rehabilitate and to need less support and
they are willing to let service users and commissioners know when
they feel that the number of hours they provide could be
reduced.
